Removal of Patients Policy

To ensure a consistent and Practice wide approach to the removal of Patients. Removal of patients will only be considered if the patient meets the criteria outline below and each removal is considered carefully prior to removal.  We will aim to always explain to a patient the reasons for being removed from the list either in writing or verbally where this is appropriate.

Patients will not be removed from the list solely because they have made a complaint against the Practice or for any discriminatory reason.  Reason for possible removal include:

  1. Physical violence to staff, Doctors or other patients
  2. Threat of violence or threatening behaviour to staff, Doctors or other patients
  3. Abusive or disruptive behaviour including when under the influence of alcohol or drugs
  4. Theft from the Surgery, staff, Doctors or other patients
  5. Criminal damage to the Surgery
  6. Dangerous dogs posing a real or potential hazard on home visits
  7. Altering documents e.g. prescriptions, insurance certificates
  8. Defamation of Doctors or staff
  9. Unreasonable and / or unjustified rude, threatening or harassing communication
  10. Misuse of appointments or misuse of home visits
  11. Moving out of the area
  12. Any other reasonable breakdown of the bond of trust and relationship between doctor and patient
  1. Patient intends to be away from the United Kingdom for a period of at least three months
  2. Patient is in the armed forces of the Crown (except in the case of a patient to which special arrangements as per NHS contract applies)
  3. Patient is serving a term of imprisonment of more than two years or more than one term of imprisonment totalling, in the aggregate, more than two years
  4. Patient has been absent from the United Kingdom for a period of more than three months
  5. Patient has died
In some cases we reserve the right to remove other members of the household.
