Online Registration

If you would like to register online, you can do so through the NHS website.

Alternatively, you can complete each of the following 3 steps   

1. Please complete this form to register with NHS GP services.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

2. We will need to check your identity and address to complete your registration.  Please bring the following down to the surgery. 

Proof of Identification: This must be an approved form of identification which is up-to-date and contains a recent photo of yourself 

  • We CAN accept: your NHS medical card, your passport, your driving licence. 
  • We CANNOT accept: student cards, national insurance number cards. 

Proof of Address This must be a recent utility bill in your name

  • We CAN accept: gas bills, electricity bills, council tax bills, water bills 
  • We CANNOT accept: mobile phone bills, landline phone bills, bank statements, credit card statements, utility bills in the name of a parent or relative who is not registered with the practice.
  • If you have just moved into the area and do not have the correct proof of address (as outlined above), we can accept a copy of your tenancy agreement or rent book provided that it is in your name and not the name of a relative.

To register children under the age of 16, one or more parent/legal guardian must be registered with the Practice.

3. It is usual practice at Chadsfield to see a GP prior to completion of your registration.

Alternatively, you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.
